JANS – CenterWell Senior Primary Care celebrated the grand opening of its first senior-focused primary care center in Jackson on Sept. 27, 2023. The new facility is located at 1220 E. Northside Dr.
CenterWell Senior Primary Care is one of the largest, fastest-growing senior-focused, value-based care providers in the country. Together with its sister brand Conviva Care Center, CenterWell Senior Primary Care delivers care to more than 270,000 seniors in nearly 260 centers as of June 30, 2023, with centers now open in 15 states and plans to open 30-50 new centers per year through 2025.

Dr. Lara Clement, CenterWell Jackson East’s physician, emphasized CenterWell’s focus on whole-person health. “CenterWell’s care model allows me to get to know my patients and build relationships with them,” Clement said. “Our longer appointment times and the care team that wraps services around our patients helps remove barriers to care and helps seniors live healthier, happier lives. CenterWell Senior Primary Care locations in Mississippi are staffed by board-certified physicians, nurse practitioners, medical assistants, and referral specialists, and patients will have easy access to care team coordinators, behavioral health specialists, socials workers, and a clinical pharmacist. The entire team has been specially trained to treat the senior population. In addition to its healthcare offerings, CenterWell facilities include activity centers for local seniors.
CenterWell Jackson East began accepting patients in August. CenterWell locations in Mississippi accept patients who have Original Medicare, as well as many Medicare Advantage health plans.
